dentin caries removal: development and validation of the method for fluorescence
Dr. Wolfgang Buchalla shows that caries removal is an important problem in restorative dentistry. There is controversy about the amount of carious dentin that must be removed before the restoration. Although controversial, it is generally accepted that bacteria in the dentin must be removed to preserve the Pulpal vitality and provide longevity to restore. Data on dentin hardness are clinically used for the diagnosis of infected dentin, but with a high degree of subjectivity. The fluorescence property of bacteria may provide a solution to the problem of discriminating the infected or not infected dentin not only in the laboratory as well as in clinical practice. This lecture will discuss the properties of healthy and infected dentin, the use of the FACE (fluorescence assisted caries excavation) and their validation. The FACE can provide to the dentist, associated with clinical examination, the distribution of bacteria in the dentin. At the end of the examination, the dentist must decide what to do with the information: remove or preserve the fabric!
defective Restorations: When and how to repair?
Annette Wiegand explains that the flaws in restorations are common problems in clinical practice. While some failures are acceptable, other defects require full or partial repair. The repair is so accepted as Exchange restore as a viable alternative for the treatment of defective restorations. The repair is still comparatively more conservative as well as potentially more accessible to the patient and faster to be realized. This lecture will address the possibilities for the treatment of defective restorations and indications for repair. Different techniques and materials for direct repair (composite resin and amalgam) and indirect (ceramic and metal) will be presented and the clinical success of this treatment option
discussed.
periodontal Therapy related to the cause of disease: success without scalpel?
Philipp Sarhmann presents inside the systematic periodontal therapy, the first step in the treatment is often called the “initial” or “pre-surgery” in the books. In this sense, this stage can be considered as a pre-treatment for surgery. However, this idea is not true and disregards the impact of non-surgical step as an effective therapy. Since the primary cause of periodontitis is the dental plaque, appropriate therapy should be the removal of harmful bacteria that are periodontium. Observing the results of non-surgical treatment accurately and meticulously, it becomes obvious that the hardest work still can be done before the dentist use the scalpel. Thus, the surgical step can be best classified as a corrective phase ” for those sites that require regenerative therapy. However, it is important to discuss if satisfactory results are being achieved without surgery, and if there is something else we can use besides curettes sharp and ultrasound. Moreover, what can be considered a success within this step of treatment and which problems are not solved without scalpel. The purpose of this lecture is to discuss some approaches that may improve the periodontal therapy related to the cause as a crucial step to positive results in the long run.
