Seville, 2011-October The Nephrology, as much of the medical specialties, is facing various factors to address medium and long term: the ageing of the population and the current economic problems, which make the national system of health (SNS) see resentful.
These problems, says Dr. Alberto MartÃnez Castelao, President of the Spanish society of Nephrology (SEN) could do that we back to the starting point of 30 years if we do not on the table formulas more imaginative and participatory on all sides. The fundamental problem is the sustainability of the system ”.
Specifically, in the case of the Nephrology, alternative treatments, dialysis and transplantation, are very expensive procedures that consume substantial resources. Chronic kidney disease (ERC) is caused by a set of pathologies which are increasing in today’s society: high blood pressure, diabetes and obesity are some of them. For this reason, says Dr. Martinez Castelao, expected an increase of patients who may have kidney damage and efforts must be directed preferably to prevention, i.e. to avoid the causes of ERC. To this we dedicate a great part of our daily activity ”.
However, the President of the society States that must be prepared to assume that future with optimism and effectiveness and to proceed with the magnificent work undertaken for years. The Spanish Nephrologists have growing reputation in the national and international media, not only in the field of kidney transplantation, but in all the varied aspects of the Nephrology ”.
In this regard, the SEN has established close contacts with primary care doctors for years and has more recently made specific and differentiated programmes with other specialties to precociously detect the ERC and vascular damage that it can be derived. Are carrying out a program that we call together in Chronic Renal disease ”, with the help of the pharmaceutical industry, to coordinate the efforts of detection and joint action in the management of the ERC with the societies of Cardiology, internal medicine, Neurology, Endocrinology, Diabetes and primary care ”.
The continuous yatrogenia remain a cause of acute renal failure
Acute renal failure has very different causes and can occur within own hospitals, due to conditions or factors that favor its presence in complicated patients with risk factors or sick clinics that are referred from primary care or other specialties.
When other doctors detected a kidney problem, in general are in immediate contact with your respective area nephrology services. Depending on the complexity of each case, these patients may require to be referred to Nephrology or handled jointly with other specialists ”, points out Dr. Martinez Castelao. It is the case, for example, of patients admitted in the units of intensive care, requiring a very complex management and in which the nephrologist has to act as support for the patient.
, However, ”, explains the expert, a particularly important aspect is that of the yatrogenia, i.e., the damage that can occur to the kidneys as a result of the use appropriate non-drugs or media Diagnostics in certain patients. This is a chapter in which we insist in the document of consensus between SEN and society Spanish medicine of family and community (SEMFYC), but remains highly topical and that not we cannot lower our guard ever, because continuing to be a cause frequent acute renal failure ”.
Studies NEPHRON and FORESEE
In the framework of the XLI Congress the SEN held on 16-19 October in Seville, Dr. Martinez Castelao refers to two investigations currently underway in the area of the Nephrology: NEPHRON projects and anticipate.
The project NEPHRON is a very important study by its dimension in the number of patients included and its duration, which is why we will soon have relevant conclusions in the broad field of the prevention of arteriosclerosis in you patient with ERC ”, said the President of the SEN.
The project FORESEE consists in the formation of the nephrologist in imaging techniques and ultrasound, these apply to the detection of diseases like arteriosclerosis and others in which the kidneys are involved. According to Dr. Martinez Castelao, is of an also a long-term project, which will have as a sequel to two courses of University expert in imaging techniques, that are going to develop in two successive stages in the UDETMA of the University of Lleida and the Hospital Ramón y Cajal in Madrid and the University of Alcaládedicated to imaging techniques in the detection of arteriosclerosis and interventional nephrology, respectively – as problems with vascular access – or kidney biopsy.
The Spanish society of Nephrology
The Spanish society of Nephrology is a scientific society whose objectives are to promote and disseminate scientific and advances clinical in the field of specialty, covering fields as diverse as high blood pressure, kidney disease, the progression of renal insufficiency, or renal dialysis (hemodialysis or peritoneal dialysis) or kidney transplant replacement therapy when the renal function does not allow the survival of the patient. It also promotes research on extra kidney complications of kidney disease (anemia, bone disorders, cardiovascular complications, etc.). It currently has 1,700 members between researchers and clinical Nephrologists on topics related to the specialty. This company organizes a Conference, as well as several refresher courses throughout the year, both for specialists and resident doctors.
