Madrid, November 2011- study Cost-effectiveness analysis of the Spanish Renal Replacement Therapy Program ” (1), recently published in the magazine Peritoneal Dialysis International, simulates four scenarios which varies the proportion of incidents that do not come by way of emergency but scheduled patients (increasing to 75%) and the number of patients on Peritoneal Dialysis (DP) (increased up to 30%)(, percentage recommended by experts).

Total cost savings varies according to the proposed scenario, but increasing the number of patients in treatment with DP up to 30%, improves survival, the number of patients that can be treated and, in addition, the total cost is less. In fact, up to 480 million euros could save over 15 years.

The study concludes that, by analysing the cost and the efficiency of various forms of renal replacement therapy (TSR), the DP get a favorable outcome and health net benefits are positive. Therefore when explores possible scenarios, the best is the one which created consultations pre-dialysis and DP increases 30% achieving greater quantity and quality of life to one lower economic cost ”, says Dr. Rafael Selgas, Coordinator of the support group for the development of the Peritoneal Dialysis in Spain (GADDPE) and Chief of service of Nephrology of Hospital Universitario La Paz. Unfortunately, in Spain the DP still underused and too few patients benefit ”, adds.

In fact, according to the article peritoneal dialysis is the best cost-effective alternative to the sustainability of the dialysis treatment ” (2), the annual average cost per patient of renal replacement therapy patients is 46,659 euros in hemodialysis and 32.432 euros in home peritoneal dialysisIE hemodialysis is 44 per cent more expensive than peritoneal dialysis ”, says Dr. Javier Arrieta, spokesman for the GADDPE and Chief of service of the University Hospital in Basurto (Bilbao).

According to the last row of the Spanish society of Nephrology (SEN), there are currently more than 48,000 patients in Spain that they suffer from chronic kidney disease at an advanced stage and were receiving some form of renal replacement therapy, representing a prevalence of 1.033 patients (the 45.7% in HD)(5% in DP and the 49.2% in transplant) per million population. The economic impact of the TSR in Spain is estimated at 1,829 million euros (3) per year.

The authors of the study conclude that, from a perspective of minimising costs, it would be favourable to increase the number of patients in DP.
