Madrid, December of 2011- the Pediatric primary care consultations have undergone a sea change in recent times. Origin psychosocial disorders (school abuse, syndrome of the Emperor …), chronic diseases, childhood obesity, food allergies and respiratory disorders, among others, have become some of the issues that occupy a large percentage of time of the consultations of primary care pediatricians.
In the last Congress of the Spanish society of Pediatric Extrahospitalaria and primary care (SEPEAP), the professional round table dealt with care overload and excessive accessibility: failure of success ”. Rapporteurs Fernando Malmierca Sánchez and Rafael Jiménez Ales respectively, addressed the issues the community-based Pediatrics for the child of the 21st century ” and the pediatrodependencia and the limitation of the quality of care ” demonstrating the need that the health care environment suits the current needs in order to provide a better service and assistance to children and Spanish teens:
1. The child should be the focus of the paediatric health care system; Spanish health system should conform to their current and future health care needs.
2. Not all the problems of child health require the same time to its proper attention, although all of them can be resolved at the same level of care.
3 Psychosocial problems at children and adolescents, as well as chronic conditions have increased in number and importance, most of them can and should be settled in primary care, but its correct assistance requires much greater time to the pediatrician, as well as a multidisciplinary work and teamwork.
4. Overload, due both to the number of patients assigned to every pediatrician, as permissiveness, including promotion of unnecessary attendance at consultations by banal processes, particularly affects children and adolescents more vulnerable, with more complex problems, and which are the most damaged by the improper use of the health system.
5. All stakeholders, including pediatricians, scientific associations of Pediatric primary care, health authorities and society itself, must help protect the well-publicised patients the good use of resources.
Spanish society of community-based Pediatrics and primary care (SEPEAP)
The Spanish society of Pediatric Extrahospitalaria and primary care (SEPEAP) emerged to accommodate a group of Pediatricians with needs specific update, training and professional problems differentiated and minusvaloradas pediatric practice 25 years ago. Scientific and professional and non-profit currently 2,660 partners.
The structure of the SEPEAP consists of societies and sections provincial and/or regional paediatric Extrahospitalaria and primary care, without losing its own character, linked to it and to the societies of Pediatric regional of the Spanish Association of Paediatrics (AEP).
Since the beginning of the SEPEAP were clear its purpose, as indicated in its statutes: promoting the study of Pediatrics Extrahospitalaria, the problems of children and adolescents within the lash (outpatient health centers, public and private clinics, hospitals external consultations, etc.). In addition, is intended to cover the three fundamental missions of Pediatrics Extrahospitalaria: care (preventive, curative and rehabilitative), teacher (collaboration in the formation of undergraduate student of medicine in Pediatrics Extrahospitalaria, in the training of residents in the specialty of Pediatrics in pediatric Extrahospitalaria)(, in the continuing education of the lash pediatrician in exercise and health education for the population at all levels) and researcher, to promote the correlation of paediatric Extrahospitalaria and services primary care with other health services and social resources of the community within the A.E.P.
